The FCPS training follows a systematic roadmap divided into two major phases: Initial Training (IMM Phase) and Final Training (Fellowship Phase), with mandatory academic and clinical milestones at each step.

- Eligibility and Enrollment
- MBBS with valid PMDC/PMC registration
- House Job completion
- Passing CPSP Entry Test (FCPS-I)
- Apply to CPSP and obtain training slot allocation
- Registration with PRCMDC and assigned supervisor
- Initial Two Years – IMM Phase (FCPS Part I to IMM)
- Structured training under supervision in the relevant department
- Maintenance of logbook and portfolio
- Active participation in academic activities: case presentations, journal clubs, mortality/morbidity meetings
- Completion of required CPSP workshops (Research Methodology, BLS, etc.)
- Submission of synopsis for research project
- Appear in Intermediate Module (IMM) exam as per CPSP schedule
- Final Three Years – Fellowship Phase (Post-IMM)
- Continuation of advanced clinical training in sub-specialty rotations
- Increased responsibility with supervised autonomy
- Regular formative assessments and DOPS/Mini-CEX
- Completion of research dissertation/thesis as per CPSP protocol
- Submission of e-logbook and record of academic activities
- Preparation for and appearance in FCPS-II examination
- Exit and Certification
- Passing FCPS-II exam (theory, TOACS, and clinical)
- Fulfillment of all academic and ethical requirements
- Final certification awarded by CPSP
